If you cannot locate your stock certificate(s), you must complete the Lost Securities Affidavit section on the back of the Exchange Form. There are three steps to completion of the Affidavit: calculation of the lost certificate bond premium and processing fee, signatures of the registered shareholders and notarization of the registered shareholders’ signatures. The completed, signed and notarized Exchange Form must be returned to Computershare along with a check for the amount of the bond premium and processing fee payable to Computershare Insurance Services.
